    A 22-year-old patient came to a thyroid ultrasound with complaints of an enlarged thyroid gland.

    On ultrasound, the thyroid gland is not enlarged, but with increased blood flow at CFM.

    About 4 hypoechoic lesion are visualized posteriorly to the thyroid gland, two upto 1 cm, one on each side and two upto 0.5 cm, on the CFM without blood flow (parathyroid gland enlargement / adenoma / hyperparathyroidism?).

    Anterior to the thyroid gland there are two paratracheal LNs up to 6 mm.

    Down to the thyroid gland there is a persistent thymus.
