Thickening of the spermatic cord in a teenager
#1

A teenager with a syndrome of edematous scrotum was admitted to the emergency department of the Children's City Clinical Hospital, and was urgently sent for an ultrasound scan.

According to the child, pain is disturbed only when the corresponding half of the scrotum is touched, the symptoms appeared a couple of days ago, noted a couple of episodes of severe pain, before the onset of attacks, according to the child's description, he unsuccessfully sat down .

On ultrasound, pronounced asymmetry of disturbing side with increased blood flow, there is a thickening and an increase in echogenicity of the spermatic cord (deferentitis / funiculitis?), which is visualized even posterior to the testicle, leading to an echo picture of an abnormal position of the testicle like bell-clapper deformity, and also a small amount of fluid around the testicle. At spectral doppler (not in the video), reliable signs of the arterial spectrum of blood flow in the testicle were not obtained, but only in the spermatic cord (incomplete testicular torsion?).

Differential diagnosis: incomplete testicular torsion, deferentitis (vasitis), funiculitis.
