  Inguinal pearl

Child 4 years-old with cremasteric reflex (testicular fearful syndrome, retractile testis, false cryptorchism).

At US one testicle in the inguinal canal, near the upper pole there is a hyperechoic small lesion to 2 mm (calcinated appendage?).

In addition to true (congenital undescended testis) cryptorchidism and the cremasteric reflex or shy testicles syndrome(retractile testis), there is also the phenomenon of the ascending testicle or acquired undescended testis, which is a complication of the retractile testis - external link

And what' s interesting that the histology of true(congenital) and acquired undescended testicles is identical - external link

Also in the WHO ultrasound manual - external link , I found a section about scrotal ultrasound, and a fig. 15.40 on page 376 Tunica granuloma (Granuloma of the tunica albuginea) with a similar echo-pattern.
